王冰：端，首也。始，初也。表，彰示也。正，斗建也。中，月半也。推，退位也。言立首气于初节之日，示斗建于月半之辰，退余闰于相望之后。是以闰之前，则气不及月；闰之后，则月不及气。故常月之制，建初立中；闰月之纪，无初无中。纵历有之，皆他节气也。故历无云某候闰某月节闰某月中也。推终之义断可知乎。故曰立端于始，表立于中，推余于终也。由斯推日成闰，故能令天度毕焉。
吴崑：“立端于始”，谓造端为历元，所谓冬至日子之半是也；“表立于中”，谓表正斗建于月中。“推余于终”，谓退闰余于积气之盈。若此，则天度虽远，而可尽知也。
《素问注释汇粹》：立，确立也。端，始也，这里指岁首。《左传。文元年》：“先王之正时也，履端于始。”注：“步历者以冬至之日为岁首。”“立端于始”，即确定冬至节为一年节气的开始。

《素问注释汇粹》：表。圭表也。《宋史。律历志》：“定时考闰，莫过于圭表。”圭表为古代天文仪器之一。中，指中气。一年二十四节气，月初为节，月中为气。《左传.文元年》：“举正于中”注：“举中气以正月”。“表正于中”，即以圭表测量日影的长短变形，计算日月的运度，来校正时令节气。推余于终，即上文积气余闰之义。谓月有余日，则归之于终，积而为闰。

吴崑：此言地以九九之会者，黄锺为万事之根本，在地则九州九野，皆以九而制会通也。“十日”，为甲、乙、丙、丁、戊、己、庚、辛、壬、癸。天有三百六十五度四分度之一，岁有三百六十五日，积气盈闰，此气数之合于天度者如此。
高士宗：气数。二十四气之常数也。
张隐庵：气数者，生五、气三之数也。

高士宗：此《灵枢。官针篇》之言，引之以明六气加临。而有盛衰虚实也。年之所加，随在泉之位，六气各有客气之相加也；气之盛衰，司天之气，有太过而盛，不及而衰也；虚实所起，因气之盛衰，而民病虚实，所由起也；必知此可谓良工，如不知此，不可以为工矣。

吴崑注本作“五运终始”。郭霭春《素问校注语译》校同，程士德、王洪图《素问注释汇粹》亦认为吴“将‘之’改作‘终’字，文义较顺。”高士宗“始”作“周而复始”解：“承上文而问五运之周而复始，既如环无端，其运气有太过不及，则何如。”类此理解，“之”不易“终”亦无不妥。
